Hidden Avatars on Pokémon Showdown! [new avatars @ #348]

I really like the remodel they did to the ash sprite! You can see it under old avatars, despite the fact it just recently started looking like this
PS creator Zarel uploads the hidden avatars (and all avatars including custom ones) for people to use, and sprite creators make the hidden avatars, and custom avatars are normally made by the person equipped equipping them (but they must get Zarel's permission before making it their avatar). As for the program I have no clue what you mean by that.
There must be an application in which you can do these avatars.
This is pixel art, some artists do avatars like these using photoshop, paint or any other program related to graphic design.
P.S: I'll edit the thread at some point with the new hidden avis, thanks to everyone for helping.

Several new hidden avatars were added!
FWIW there is now http://pokemonshowdown.com/sprites/trainers which lists all of the non-custom trainer sprites available and which will automatically update as new ones are added.

They aren't actually avatars on PS but what do you think about these? I've felt they've been missing for a while and prefer them over the N avatar we currently have.

Custom avatars can pretty much be from any games / books or whatever til they follow a few rules (size and things like this) . In this case users are free to propose any avatars. If you want to get a custom avy you need to hold 5 badges on Smogon / be a g driver or more / some tours may give you a custom if you win them.
When it comes to other avatar I think they need to be from official video games and they will be usable by any showdown users.
