Hello Antimattle. It is evident from your team that you're very new to the Gen VI Ubers metagame. Your team needs quite a bit of rework.
1. Funny how you have both Primal Groudon AND Ho-Oh yet claim GeoXern to be your biggest threat. In XY, GeoXern is such a ubiquitous sweeper, but in OR/AS it ran into Primal Groudon which is a very excellent check that accomplishes many other things. If you still have trouble dealing with GeoXern, consider running a Specially Defensive version of Primal Groudon. At maximum investment, Primal Groudon takes only 56.4% maximum from +2 Moonblast (thanks to Fire resisting Fairy) and 71.7% from Hidden Power Ground or Focus Blast. That means you can switch in when Xerneas uses Geomancy, take a hit and use Roar on it. It's going to be very hard for Xerneas to set up again once its Power Herb is consumed. The added special bulk also makes switching into Primal Kyogre's Ice Beam and Hidden Power Ground a lot less painful. Groudon's physical bulk and offensive power are also very respectable even when uninvested.
If you want to stick with an offensive PDon, then having Bulk Up alongside Rock Polish is a waste. Rock Polish already allows you to outspeed pretty much the entire unboosted tier. You should be using that speed to hit things as hard as possible, instead of banking on the added Phys. Def to tank hits (PDon has no recovery, tanking unnecessary hits in general is a bad idea). Also, a Bulk Up set should be maximizing Sp. Def. investment, as the Physical side can use the +1 boost while specially with only base 90 Sp. Def, Primal Groudon is quite vulnerable.
2. Rayquaza's current moves are puzzling. Waterfall has no notable niche (Water hits many thing neutrally but doesn't hit enough things super effectively to warrant a spot as a coverage move for most sets); Outrage is a dangerous move to lock yourself into, particularly if there is a Fairy type on your opponent's team. I strongly recommend using Dragon Ascent as STAB and Earthquake as primary coverage. These two moves together has nearly unrivaled coverage.
3. Heracross is a very suboptimal Pokemon in Ubers and has no real niche. Its sole purpose is to counter Darkrai, and doesn't accomplish much else. There are Ubers Pokemon that handle Darkrai very well. Choice Band Ho-Oh with Sleep Talk is a very notable counter, as all of its attacking moves (Brave Bird, Sacred Fire and Earthquake) will OHKO Darkrai when backed by CB. Choice Scarf Xerneas with Sleep Talk, Moonblast, Megahorn and Focus Blast / Close Combat also accomplishes the same.
4. There isn't any reason to run Bulldoze over Earthquake on Ho-Oh, or Pressure over Regenerator. You're also wasting an item slot with Focus Sash, as Ho-Oh has fantastic bulk that should be utilized. Focus Sash is also an extremely weak choice with no Defog support. At 248 HP / 52 SpD, Ho-Oh can survive anything from a +2 GeoXern (even Thunder) when SR is not up, and hit back hard. Another possible set is the previously mentioned Choice Band set, that has the added bonus of checking Darkrai.
If you opt for specially defensive PDon, then it can use Stealth Rock. Otherwise, you need to replace Palkia and Kyurem-B with two of the above, juggling to achieve these things at the same time: Defog support, Stealth Rock, a check to Extreme Killer Arceus and a switch-in to Primal Kyogre. Kyurem-B is a Pokemon better left in OU; Palkia with Assault Vest is just bad. You need to be either full on offensive with your Palkia (maybe Choice Specs), or you need to run Rest + Leftovers to be able to repeatedly come in on Kyogre without getting worn down. Also, for Kyruem-B, Zen Headbutt and Payback are both terrible coverage options, as they don't hit super effectively anything that your STAB does not already hit at least neutral. Your moves on Palkia are also suboptimal: the four attacks that Palkia should ever be running are Dragon (Spacial Rend, maybe Draco Meteor), Water (Hydro Pump; Surf if you don't like the accuracy or Scald if used on defensive sets), Electric to hit Kyogre (Thunder. Kyogre brings rain, so there's no reason to use Thunderbolt when Thunder has 100% accuracy) and Fire to nail Steel types (Fire Blast).
